You have a terminal cancer. You will die within a month from it. However, there is a treatment that will cure you completely.

As with all medicines, there is a side effect. Once a month, you will turn into a werewolf and go on a rampage throughout the city. Killing anyone in your path without mercy, destroying anything you can. This will last for one night, and you will have no recollection of it. You also don't know when during the month it will happen, it is always completely random. You will know that the murder and rampage you see on the news is on your hands.

Do you take the cure?

No, because when you break it down you're saying, would you rather countless numbers of innocent people to die while you live, or would you rather die and let everyone else live, as a result of a yes or no question. I know that I definitely wouldn't be able to live with myself for making a decision like that, like if you bought your kid a bike you knew was faulty and unsafe, and his brakes didn't work and he fell off a big cliff into some sharp rocks.

Does become a tough choice. Mortality seems to be one of man's biggest fears.

But

To be cured and to bring about such useless destruction at the cost of possibly a few more days or weeks, maybe even a month, because tomorrow is never really guaranteed to us.

i'd rather die than let another person die in most cases. Certain people escape that clause obviously. Yeah i'd let myself die instead of millions of random people.

Well since we're all going to die anyway I might as well extend my death date and take the cure. Sure a lot of people would die sooner but I'm selfish.

Whatever you can have aids or herpes or syphilis or whatever. It's just the fact that you have some kind of terminal disease that will, 100% probability, kill you within a month. Maybe we can call it Ebola, with a higher death rate.

I would have someone follow me who would tranquilize me every time I start to turn into a werewolf. Assuming that it'll happen like in the movies and that there is an effective tranquilizer for such a beast.

To keep people from trying to come up with "loopholes" let's change the argument to be a bit more "unbelievable" (assuming turning into a werewolf is believable):

If you take this treatment, once a month, every month, 100 people will die, 100 homes will be destroyed, and 100 business will be smashed to pieces. BECAUSE OF YOU.
